If you're going to insert a screwdriver handle into a vagina and wang the end around should the ratchet be set in the anticlockwise or clockwise direction, or be disabled completely?

Disabled completely is probably the best way so you are both prepared for those times when you can't get your hands/vagina on a ratchet screwdriver.

Otherwise it's down to personal preference.
