Yes - if it portrays the act in a "realistic way", and a "reasonable person looking at the image would think that any such person or animal was real."

In staged cases, usually the people would obviously be real anyway, so it's just whether the act is portrayed realistically.

Note that extracts from BBFC films are _not_ exempted from the law - further suggestioning to me the idea that a "realistic" fictional image can be covered by the law, even if we could prove (using information other than what the image itself looks like) it was fake.

Yes...discussed this phenomenon at the recent Onscenity conference. However, there are even bods out there who regard reaction vids with suspicion - as this story demonstrates:
